Note: It can take up to 10 business days after the crash date before the accident report is available through the online portal.

  2. Your Address
  3. Driver’s Full Name

  9. Name of Fatalities (if any occurred)

For payment, the ALEA accepts either cashier’s check, certified check, or money order in the amount of $15.00 payable to the Alabama Law Enforcement Agency. They will not accept cash or personal checks. 

Note: Be sure to include a self-addressed, stamped envelope with your completed form and payment.
